DOI: 10.1046/j.1464-410X.1998.00921.x AbstractOrthotopic bladder substitution in females who have tumour involving the bladder neck is not usually acceptable because there is a high risk of recurrence in the remnant urethra. We describe a procedure of bladder orthotopic substitution featuring the removal of the urethral mucosa and submucosa and their substitution with ileal homologues to avoid or minimize urethral recurrence. The absence of anastomotic sutures between the neobladder and urethra improves voiding, as hypercontinence is otherwise a major problem.
